Notify your employer
Report the accident as soon as possible. The employer must fill out Form CFSE-373 with the details of the incident.

Seek medical attention
If it was an emergency, go to the nearest emergency room. Save the medical record to submit when filing your case.
Gather the necessary documents
Take with you: Form CFSE-373 (if you have one), Official ID, Initial Medical Record, Health Insurance Card (if applicable).
Visit a SIFC location
Go to a regional office of the CFSE or the Industrial Hospital to formalize your claim.

Complete the additional forms
You can sign at the office: Consent Form for Medical Treatment (CFSE-1219) and Affidavit (CFSE-450.1), if your employer did not submit the CFSE-373.
